I think a general form of 'joining duty' would suffice. Here you have to segregate the nature of absence into normal leave, leave of absence due to lockdown. A general format would serve your purpose if the absence is authorized either as regular leave or Covid-19 lockdown. What's this police case you are talking about? Is this anything to do with employees being caught and quarantined for violations of SOPs imposed during the pandemic regulations stipulated from time to time? If yes, then you have to think it over further on such cases in detail individually. Maybe you have to insist on a Covid Negative certificate before joining.
