In your case, what matters more is the maturity of the subordinates. Are they capable of appraising their superiors? Has any training been conducted on how to conduct the performance appraisal of superiors? Is it that you wanted to bring 360-degree appraisal to your company?
There are always perception gaps between superiors and subordinates. What efforts have you taken to bridge those gaps? I request you to concentrate on these larger issues. The appraisal form is something smaller. The appraisal form can be the same; it need not be different.
