Hello Abhi,
Just a suggestion from my side: You can assign weights to the various parameters. This will help you in attaching relative importance to the various job responsibilities. I am sure you are aware of this weighted appraisal system; however, you can also do without the weights.
In the "Critical attributes" section, you have mentioned "Job Knowledge, Skills, Competencies & Application"... What competencies are you referring to? It would be better if you could have been a little more elaborate.
You could also have broken up the "Leadership Skills" into various subheads for measuring various leadership aspects.
