Our company is running with 2 branches in Chennai and having 2 separate registration address, now we combined both into one and shifted to another place. What is the procedure to change the address in ESIC and EPF?

You need to write an letter regarding the same to both department along with the proof of changed address, signature of authorised signatory as per PF & ESIC department. Apart from that you need to submit for form Revised 5A (Online and Off Line as well).
Please check with both department as well.
