In Maharashtra, similar format is provided for contract labour
Plus there is a format specifically provided for security agencies under Private Security Agencies Regulation Act
I would like seniors to tell us if other states have any formats specified.

Madam, All the suggestions are really good. When you outsource a party for getting ur ID card along with biometric reading machine or punching machine, they will be having the formats. just check out the formats and get the cards developed

where is the attahcement of Go 25 c if you have please send the same

You can add the followings:
a) Give separate color of ID card if some of the departments not allowed to enter into construction site.
b) Write the instructions related to misuse of the card.
