In most of the small and medium, as well as single-ownership companies, there are no systems and procedures. The accounts guy doubles up as an HR guy, the admin guy doubles up as an HR guy! Most of these people do not know the basics or fundamental rules and regulations. Even the management is not keen on having such systems in place as their absence gives them enough leverage to play around with people, using them as pawns to pit one against the other.
In the above company, if we inquire, we will come to know a lot of dirt and garbage. But still life goes on, business goes on... who cares!!!
