I got one file contain more than 500 different type of pre-written letter format, I have download this from citihr site only, once again uploading the same for new citihr members & other who miss this.
Do download it, it will help u a lot to write the basic letters, will give u idea how to start the letter and will help in yr day to day letter drafting.
Do give me yr feedback about the same.

Dear all,
Can anyone guide in this below matter,
We had one small company and it was closed during 2000, personal hearing was posed by ESI on various dates we attended all the hearing and finally order u/s 45 A was given for the period from 95-96 to 99-2000 and we paid all the contributions including interest & damages. Now we have received a public notice in the newspapers saying that we haven’t attended the Personal hearing for the period 93-94 & 94-95. Is it possible for the corporation to insist us to produce the records for the left out period? Since this is the mistake of the assessing officer for not going through the file while assessing from 95-96 to 99-2000. What should we do in this matter? Moreover the records pertaining to the period 93-94 & 94-95 is totally damaged. How to reply ESI? Please give us your valuable suggestion.
Thanks & REgards,

Dear Mubarack,
Before any reply, we need the following information. Have you received any notice for the period 93-94 and 94-95.If you have recd the notice it is obligatory on your part to attend the personal hearing and give an appropriate reply. The department has no authority to call for very old records.Anyway approach the authorities and state the factual position.

Dear Mr.Janardhanan,
Thank you very much for your reply. Since I have joined now, I am not sure whether ex-incharge has received the notice for the period 93-94 & 94-95 or not, but when verified with the ESI office they are showing the inspector report and a copy of letter posted to us during 1997. My question is that, is it not the duty of the assessing officer to verify the file and issue the demand along with demand raised for the period from 95-96 to 99-2000. If we accept to pay the amount mentioned in newspaper then the corporation will ask us to pay interest and damages with separate notice. How to handle this case? Your valuable suggestion will be highly appreciated.
