In cases where the maintenance team is maintaining the checklist, they should be held liable.

This letter is to inform you that a serious complaint has raised against you due to recent accident that occurred scaffold board broken where lack of proper fall protection and safety and caution are reasoned for.
It has been observed by the management that due to your negligence/failure of rectifying the defective board whenever maintaining the scaffold and regular routine check caused the accident. Moreover, question raised either you’ve ensured the ineffective fall protection or not.
However, in view of your admission of your act and verbal assurance that you will not give room for such or similar instances in future, you are hereby let off with WARNING.
Besides, since your unpreparedness for safety caution indicates negligence of work ; will be recorded in your yearly performance evaluation. It is hoped that you will take good advantage of the leniency shown to you and you will strictly keep up to your assurances.
