Just fill up the necessary form, sign the form, and also attach your updated bank passbook/statement (Account must be with any of the nationalized banks) zerox copy to receive a PF Amount directly credited to your bank account to be given to your new/current employer.

For withdrawing PF, you also need to submit Form 10C (Employee Pension Scheme) along with Form 19 to withdraw the whole amount of deposited PF (12% of employer's share).
