Dear PROFESSIONAL,
Before you submit your application for a License, you require the following papers:
1) A copy of the contract/work order for contract work, a written agreement of Rs. 100/- STAMP PAPER between you and the Employer.
2) Your address proof/PAN No.
3) An undertaking from the Employer (that he shall follow all provisions of the act).
4) Duly filled challan towards security deposit and license fee (This is available in the Labor office). Only after you have deposited the fees in RBI or treasury.
5) An amendment request from the Employer requesting an amendment in the CERTIFICATE OF REGISTRATION.
With all the above documents, you can apply for a Labor License under the Contract Labor Act.

Please go through Chapter IV for the grant of a license if you are employing 20 or more contract laborers or have employed 20 or more contract workers on any day during the preceding 12 months. Obtain Form-V from the establishment/principal employer where you want to employ contract labor. Apply in Form-IV to the Licensing Officer (DLC/ALC) of the concerned area, together with the appropriate fee and security deposit (varies from state to state) as prescribed, along with the required documents specified by the licensing office.
