"Where was the hole watch from safety side when it was declared as a confined space??????Was he/she enjoying somewhere inside the plant or slept on pipe racks at the time of incident ?????" What you wish to prove by saying such a statement?

Is that the responsibility of a safety pro to stand round the clock in the work premises and monitoring that all are going well or not?

System failure is one of the root cause as you rightly said at the end... "This incident shows the total failure of contractor's HSE system"... HSE system does'nt means only the failure of a Safety Pro... If in a plant, if there is only one or two safety pro, is that possible to him to stand on and monitor all the works which is going inside the plant... Perosn should not work, system should work... Also if you go deeply into particular incident you can see all the physical, human as well as system cause which resulted into this multiple fatality...

No contractor will shown interest in working so systematically and safetly if there is no demand from the Client Side... They will always shows negligence... If you know any contractor who is dedicatedly following all safety norms in India please let me know???

Dear Borntosin,
Safety is not only responsibility of safety personel only, It is everyone's responsibility, so we cannot just blame the HSE for it.
If there were loop holes & who so ever has missed it, it is everyone's responsibility to look into it.
This insident is not to debet on its loop holes, It is to take pro-active actions for future so that such recurrances shouldnt occur.
