You have not indicated who you are (a student, a researcher, etc.). Additionally, you have not provided an introduction explaining the purpose of the survey and how it will benefit the respondents.
If I may take this opportunity to inform other researchers, the questionnaire should have been piloted before being published here for responses. For instance, starting with personal questions can deter respondents. I typically refrain from disclosing personal information in surveys that are not well-designed and do not specify who is collecting the data and for what purpose, despite your note on confidentiality.
Moreover, requesting respondents to use a different colored font may discourage participation. Instead of a narrative format, using a table for Likert-type questions would enhance the layout and facilitate responses by allowing respondents to tick boxes.
These are my initial observations.
