In the form where at the starting you have asked the person to tick the relevant points, instead of that, keep it as a scaling process from 1-5. For example, let a person mark the points as 1 for salary or 2 for something else. 5 means the strongest reason, and 1 means the lowest reason. This way, it would even be helpful for you in judging which was his most important reason for leaving the job.
I have one format at home that follows the same method. Let me know if you want it for guidance.
