I need to design a survey form of about 20 question that I will send out into my organisation to get feedback from people about the possibility of introducing a Performance Appraisal System into the organisation.

There are two parts of the survey.
1. The design
2. Administration
Design:

If you want a customized one for your company, Eduquity (
) may help you in creating the same.
(at a charge :-)
Administration:
Send a printed paper to all the target group and collect the same after they fill in.
Alternatively, I can tech enable the same online ( for a small charge) so that you can send the URL (link) to the participants and you can see the results as they fill in!
