I too agree with the reason "D". I have myself seen an executive getting paid 28K and asst manager being paid 16K in the same dept.

Dear HR Veterans,
Thank you for this question and i hope these might be the reason's why employee's quit the job in my perception
firstly it's the Boss Many employees do not leave the job/organization they only Leave the Boss so ultimately if the relationship between Boss & Sub-ordinate is not good they may quit
Secondly it's Inequitable salary Distribution
Thirdly Employee's look for Better prospective (Promotion,s. Designation,s and so on)
Fourthly it's Due to Work Stress, Work load, Change of Work Environment...
Fifthly it comes the recognition
Last is the Personal problems such as Travelling, Family problems....

I am sorry Ram,
I am not agree with you as the boss does not matter, it is only important some time like if the boss is behaving in wrong manner or using wrong words or giving pressure which are not require but most of the time boss do not have any interaction with employees.
Most important if you want to retain employees in the organization, you need to pay them good salary. As now a days money is the motivating factor and as much as you pay, employee will be ready to work as you want. Job satisfaction is also important but this is the second important factor. If we these two points batter in our organization employees will not leave the organization.
Thanks,
