Thanks Shijit for posting. I have looked at the pdf document and found it strange that the scale used is skewed towards the positve responses (3 positive and one negative). It's preferable to use a balanced one.

its good questionnaire. i want to comment that please add "recognistions or rewards" that he/she received or any motivations in terms of monentary or non monentary.
