Note: Letter is not required. Just u go there and tell them your PF No. they will issue the both the forms.

Hi Manmitha,
Please check at the court campus there is standard and printed format avalable(If I am right it's Form 19 A & C) you have to fill and submit it through your employer, after her comment then only PF commissioner will act on it. If employer is not acting then you can apply directly with a copy of proof to PF commissioner,

There is no need to write a letter to pf authorities requesting pf forms. Claim forms (19 &10C) may be downloaded from the website : www.epfindia.com and the same may be filled and sent to pf authorities.
you are required the following for claim :
s/b account no
revenue stamps 4 Nos (1 Rupee)
Your PF number and brnach.
Any other help, you may reach me at 9445263172
