The answer to your question varies with the business of the company. For example, in a software company, the following are the exit formalities the employee has to fulfill in order to get relieved from the company:
1) Clearance from the project incharges (about delegation & transfer of the specific module or the project they are in)
2) Clearance from the System Administrator
3) Clearance from the Operations Department
4) Clearance from the Accounts Department
5) Exit interview form, and respective HR to conduct the exit interview
6) Informing the security and other applicable teams regarding the relieving of the employee
You will have to highlight each point as a subheading and then list the required points to fulfill them as a whole.

1. Employee working-related, which means clearance from immediate supervisor or HOD.
2. General departments like Personnel, PF, Statutory Nomination Forms are completed, Accounts, Loans & Advances, Finance, Security Deposit & Lease, Payroll and HR, Dues and deletion from payroll, Legal, Removal of name from Signatory status and necessary changes, and Banking, removal from Bank Signatory & Treasury.
IT, Email ID, and other software access removed.
Administration, Swipe card, Mediclaim card, etc., taken from the employee.
Based on these premises, the details can be worked out.
