I saw your inquiry about Performance Evaluation for factory workers. Let me introduce myself first. I am working in the HR Department of a company based in Dubai. I had the opportunity to conduct evaluations for the last year, from January 2011 to December 2011, for both executive-class employees and factory workers. In short, I have a template or you can say the whole form for this. I am willing to assist you, but you also need to help me. Currently, my GM has tasked me with restructuring the entire organizational structure of our company. We have a total headcount of 87 employees. Please assist me in this regard.

It appears that you are exchanging information based on your requirements. Concerning your GM's requirement, have you had discussions with him to seek his inputs/ideas? If not, please schedule an appointment with a specific agenda. Your GM needs to clarify the reason, business objectives, and purpose associated with the expected outcomes for the organization's restructuring; otherwise, it will be challenging for you to proceed.
