I want to conduct an employee satisfaction survey in our company. The main aim is to find the reason for the increasing attrition rate in my company. It's an IT company with 50 employees. Despite giving good hikes during appraisals due to the growth in the market, the attrition rate is still increasing. Therefore, my management wants to identify the reasons behind this trend so that the issues can be resolved.
Please assist me in creating an employee satisfaction survey for the aforementioned purpose.

Thanks, Avika and Chitra. An employee satisfaction survey should be done by a third party and not the in-house HR. Even if the process is run by the HR Team, it still needs to be audited by the external team.

The challenge with your company is that it falls within the SME category. Yours is not even a medium-sized but a small company. The reasons for attrition are within your post as well. You have written, "Considering the growth in the market, we are giving good hikes during appraisals but even then the attrition rate is increasing."
Dear friend Keerat, try to understand, people don't work for money alone. By giving good hikes, you cannot buy people. What people expect is a good working environment. A good environment is not just the physical space but also the psychological environment. Attrition will continue to happen as long as there is no:
a) culture of empowerment
b) culture of inclusion (each and every member must feel that he/she is part of the decision-making process)
c) culture of learning (this will come provided higher management spends 'x' hours every year on learning)
d) culture of change
e) culture of collaboration
f) culture of self-discipline
g) culture of competition
h) culture of recognition
i) culture of innovation
I am of the opinion that in reality, you may not require ESS. In most of the vernacular languages of India, there is a parallel proverb to the Hindi proverb "Haath ke kangan ko aarsi kyon?". ESS is something like that.

Some of my learnings from conducting 20+ ESSs so far.
1. Perceived confidentiality & Objectivity: You would be surprised how people give completely different answers based on their perception of confidentiality & objectivity. ESS should be conducted by a 3rd party to eliminate this risk.
2. Difference between data and insights: ESS is not just about excel sheets, charts, and graphs. For example, 47% of employees are happy about the canteen has no meaning by itself. The ability to correlate different dimensions is far more critical. That's called "insights." You need a person with vast diverse experience to make those insightful inferences.
3. "Unintended consequences": With ESS, you will be raising the expectation bar tremendously. Work on a plan, commit resources to improve HR systems accordingly. Poorly executed ESS leads to more dissatisfaction.
4. Perceived CEO ownership: Employees generally know if it's an HR project or a CEO project. The outcome would be different as per the perceived owner.
ESS is a great cost-effective way to improve HR systems and avoid surprises.
