You need to fill the below form and submit to your current employer for transfer. Form 13 is used to apply for a PF Transfer

For Pf transfer, you do not require the consent of your old employer. You can submit Form 13 to your present employer and he will submit the same to the PF authorities, if a 3 A is required, again this will be send to the previous employer.

Hi,
Pf Transfer Form is to submitted in Duplicate, as one form is sent to the Old employer similarly if the transfers are from more than one account and if withdrawl is to be taken please see to it that all the transfers are taken care of.
