Company name changed to Pvt Ltd ( Require Appointment Letter format)

Dear all,
i need help regarding Appointment letter format , as Company name changed to Pvt Ltd , So i want to give appoint ment letter to employee by new name of Company, What will be the exact steps for this process and format of Appoint letter.
