PF & ESIC benefits to the family members of a deceased

One of our drivers engaged through a manpower agency has committed suicide. He has the Provident Fund & ESIC enrollment number and has been paying his monthly contributions regularly. The dependents of the deceased have submitted his death certificate and legal heir certificate through the manpower supply agency. To claim his PF deposited amount, EDLI dues, family pension, and any other benefits from ESIC, what procedures need to be followed? Please guide us.

@Sitansu, you need to fill up EPF Composite Claim Form (Form 20 for PF payment, Form 10D for pension, Form 5IF for EDLI) and have it signed by the employer along with the claim documents. Then, submit it to the EPFO office.

For ESIC, you need to fill up the funeral expenses form, have it signed by the employer, and submit it to the respective ESI Office.
