Scheme Certificate need to add to current UAN

Dear Sir/Madam
I have scheme certificate from EPFO for the service period 15-07-1995 to 31-03-2006. Now I am working with other organisation from 01-04-2006 to till date. I have registered with UAN with current employer.
I would like to add my Scheme Certificate to current UAN.
Please let me k now the process
