How to apply for 75% EPF/EPS Online withdrawal after 1 month of relieving

How to apply for 75% EPF/EPS Online withdrawal after 1 month of relieving the job as date of entry/Mark exit can be entered only after 2 months in the EPFO site?
Would the Online form still accept it and process it without DOE?
If yes, which form and sub category should be used?

At the field asking for amount, should I enter 75% of PF+Pension contribution of employer as well or is it only for PF total part?
And, should I consider the tax 10% part in the calculation?
