How to amend wrong appointment date entry in the ESIC portal - have got a show cause notice also from ESIC

I have somehow entered the date of the appointment wrongly in the ESIC portal.
Please advise how I could change the details. I have got a show-cause notice also from ESIC- please let me know what I should do next.
