Help Required to draft reply of show cause notice in government department for leave without permission

Dear All members,
My wife works in the government of India (Hospital in Delhi) as staff and further she was not able to reach office some day due to the transportation issue /not availability due to the lock down.
Since we stay in Ghaziabad , and she dont know the driving she genuinly failed to attend the office. however she was able to join office with help of lift by employees going from that area .
Now her boss a lady as to justify her ego send show cause notice through the admin department.
We want to reply for that.

Was the absence one time or is it repeated over period of time?
Issues do take place in lockdown period.
Was telephonic contact made with hospital that employee is stuck due to lockdown and no transport?
