Performance Review/Appraisal - form where employee of field as well as credit can assess them selves and can be assessed by their supervisor?

Hey All, I am seeking help on format/Template for performance appraisal by Self(employee) , Supervisor & Management.
I am working for debt syndication advisory firm with staff of 75. Major employees are on field job and rest are for credit department.
Looking for form where employee of field as well credit can assess them selves, and can be assessed by their supervisor as well by Management separately.

For Each Employed Person, whether at Fixed Workplace or on-sites-mobile, at least Script Five Key Result Performance Areas/Parameters and Schedule Performance-Evaluate Every Quarter.
