Ctc breakup - structure his salary so that he will be out of ESIC?

I have some queries related to CTC.

Suppose X is getting 22000 as monthly CTC. He is not willing to take ESIC facilities. How can I structure his salary so that he will be out of ESIC?

Please check the format if it is correct or wrong:

1) Basic - 30% of CTC

2) HRA - 40% of basic

3) Medical Allowance - 1250

4) Conveyance Allowance - 1600

5) Mobile Allowance - 400

6) Special Allowance

Gross salary = 1 + 2 + 3 + 4 + 5

Gross salary - 12% of Basic as employee's contribution - P.T. = Net take-home

Gross salary + 13.16% of Basic as employer's contribution = CTC

Since the gross salary is 22k, he is not covered by ESIC.
