Please help me to ask my boss for salary increment

Please help me to ask my boss for a salary increment. Actually, in July 2017, I met with an accident due to that, I had resigned from my job. Again, in July 2018, I rejoined and it has nearly been 1 year now. Before I resigned, my salary was 15,000/m, and they are still giving the same. I want to ask my boss personally.

Instead of writing an email, meet your boss personally. Before you go, do a bit of homework, like highlighting your achievements (providing data to support why you should be given a raise). Most importantly, meet with the person in charge when they are in a good mood.

As Amby mentioned in the post, draft your achievements in the appraisal period. If they are really above expectations, go ahead and talk to him over the phone. If he responds positively, act accordingly and wait for it.

Thank you for the reply. As I said, I can't talk to him directly or through a call. I can only request him through an email. Please advise. I need a simple format to request my boss.
