ESIC - Is there is any criteria that if ESIC hospital is situated within a radius?

We want to know when contibution for ESIC is to be deucted to from the workers and to be contributed by the employer??
In case the ESIC hospital is not there at the place of working but it is there in the District, is the ESIC contribution to be deducted?
Is there is any criteria that if ESIC hospital is situated within a radius of ..............(Some) KM then the ESIC contribution to that particular site is applicable.
