Have you designed Policy on Performance Appraisal (PA)? If yes, then have you mentioned in the policy, who will conduct the PA, when it will be conducted and how it will be conducted etc?
Why you would like to have questionnaire on the appraisal cycle and to whom will it be circulated? Designing the policies is the job of the management. In that case, why you would like to circulate the questionnaire?
