Hi Sanjay,
1.There is no higher limit of PF Contribution PF rate is 12% form both side employee and employer ok.
2.Yes, there is a time limit if employee leave company withinh 3months he/she is not liable to claim the PF amount or if he/she leave company after 6months he/she 'll be get there full amount of PF
3.You have to fill 2 forms for EPF that is Form19 and Form10c for enroll in EPF Scheme
