Train Accident While Coming To Office

Dear Sir,
kindly let me know the Accident register procedure under ESIC.
what would the process under ESIC if an employee met with an accident while coming to office.

Dear STPL,
First you fill up form 12 within 48 hr. which is available on ESIC employer site & take 2 witness letter & EC copies for current period then visit to ESIC office with all supporting documents & explain them every thing about accident day, time, place.
Further they will assist you about register Form II muster roll, wages register,Accident register, covering Letter.
