HR Audit Questionnaire For Performance Management Process

I need to understand what questions are asked by auditors during an HR audit for the performance management process and also for career planning.

In the audit of a Performance Management System (PMS), the following questions must be asked:

1) Is the appraisal system user-friendly and acceptable to both superiors and employees?

2) Is the result of the appraisal system credible and believable?

3) Is the appraisal system and its results legally defensible if a legal question arises out of its results, especially on heavy sanctions like dismissal?

4) Does the appraisal system provide an opportunity for employees to recover from previous ratings?

My curiosity is, how can the answers to these questions be justified? What types of direct/indirect evidence must the auditor be looking for?
