Employee Self Transport & Safety Acknowledgement Form

Dear Seniors,
I require a format on "Employee Declaration On Safety & Self Transport" as company is not providing any transport facility, employees need to sign a form stating that they will come on own transport & company is not liable or held responsible for Mishap while commuting to office.
Please share me a format, if you have any.

Dear Suhail,
As far as I know, this kind of declaration should be illegal. An employee meeting with an accident while coming to work can be treated as 'accident during the course of work' and henceforth employer cannot run away from his responsibilities.
Hope you consult with some more people and do not do anything which is against professional ethics and legal systems of the country.

Employee is staying out of boundary location as per the company policy, but employee want to avail the nodal pickup and drop from midway.
