Employee Self Transport & Safety Acknowledgement Form

Dear Seniors,

I require a format on "Employee Declaration On Safety & Self Transport" as the company is not providing any transport facility. Employees need to sign a form stating that they will come on their own transport and the company is not liable or held responsible for mishaps while commuting to the office.

Please share with me a format if you have any.

As far as I know, this kind of declaration should be illegal. An employee meeting with an accident while coming to work can be treated as 'accident during the course of work' and henceforth, the employer cannot run away from his responsibilities. Hope you consult with some more people and do not do anything which is against professional ethics and legal systems of the country.
