Is ESIC Facility Available After Salary Cross 15000/- Pm And Beneficiary Option For Family ?

As we know, ESIC facility is not available after the salary reaches Rs. 15,001 and onwards. However, if an employee requests to continue ESIC with their own contribution, what are the rules, and does their family also get the coverage? Please reply to inform others.
