Is ESIC Facility Available After Salary Cross 15000/- Pm And Beneficiary Option For Family ?

Dear sir/madam,
As we know esic facility not available after salary reaches 15001/- and onward but if employee request to continue esic with his /her own contribution then what is the rule and is their family also get the coverage ?
