Research and questionnaire for project- How can gamification impact employee engagement

I am an MA human resource student doing my summer project on Employee engagement (How Gamification make Can Impact Employee Engagement). I wanted some help regarding the questionnaire which would be asked to the HR managers regarding their engagement practices.
Please let me know how to go about the questionnaire.

I am not a HR expert or had come across the term "Gamification", till I received your message. I am sure that had you taken the trouble to search the web, you would have found the same information. Hence, I take this opportunity to appeal to members to search the web first, formulate the aim and objectives and then seek help. This step will help me and other experts to give precise answers.

As it was the first time that I came across the ABI Award, I checked the web and found some information about it at American Biographical Institute - Wikipedia, the free encyclopedia I was surprised to find that unlike other awards which may pay the recipient of the award, the recipient has to pay to get the award.

I have heard people saying, honours, degrees (Doctorate), titles and why even naming a square or a street in a person's name is possible buying them for money, especially in USA. I think the ABI Award could be counted one among them I suppose.
