Format Of Appointment Letter

Hi,
I worked in a company which was not registered. I was getting the salary of 15000/- pm i need a format of appointment letter without ESI and PF to join a different company..

I worked as a HR Executive.. I need an attachment of offer letter wit cc breakup of 15000/- but without ESI and PF deduction.. I even need to know how to calculate ctc breakup without ESI and PF deduction..
