How To Write Handover Notes?

I'l be leaving my current job soon and would like to prepare a hand over report for the person that will take over from me. Kindly assist me with samples of hand over reports. I have worked with the organization for 2 years as maternal and Newborn health specialist and
by now I am resigning because of other opportunity.

Since you were into healthcare department, handover becomes of utmost importance. You can put into excel the names of clients you've handled and what their queries/complains were, what actions were taken, how did it help them, etc.
Ofcourse, I do not have technical knowledge of your industry, but this would be an overview.
