Letter To Bank For Converting Savings Account To Salary Account

I need a sample letter from company HR for getting my savings account to be converted to salary account.

PFA format for such a letter. If employee has changed job then you need to mention name of previous employer.

You can directly visit bank for same.
They will provide form for this to fill up and submit and also for submitting signature proof.
