Addition Of Dependent In ESI

My ESI have deducted till march 2012. On 4th April 2012, my mother has diagnosed with Ovarian Cancer. When i Checked, my mother name was not there in my dependent list.
My Employer said that i can take benefit till October 2012 but my mother\'s name can not be added now in my dependent list.
My question is: Can i add my mother name in my dependent list now?
And, another question is that , I remember that i\'ve written my mother\'s name in my dependent list but employer is denying from this..So, from anyhow can i check that i\'ve written it or not?
Please reply..it\'s very urgent..coz expenses for operation and other is very high and not affordable by me..

it is first priority of HR dept. to Add dependent of their employees as soon as employee joined if he is eligible for ESIC so right now also you can add your dependent from HR concern person of your organization
& yes amit is given you correct information that If employer is not listening you then go to the nearest esic regional office and there you can contact to esic inspectior or any employee of esic for adding the name of your mother

I should mention one more thing that from 1st April, my salary is crossing the ESI limit i.e. 15000. That's why i am asking that can i add now...

I want to change the date of birth of my dependent mother in the ESIC. How can I change the date of birth? Do I need to go to the ESIC website? Under which menu option can I make this change?

It seems your HR department does not know the procedure for adding your mother's name to your ESIC card. It is a matter of 5 minutes, and they can add it online and provide you with a print. You can talk to them and provide them with my number, 9810557181. I will explain to them how to add the name to the ESIC card. Please take a joint photo and get it pasted on the card.

I have an ESI card with my photo alone. I thought to register for family. I filled out a form, affixed a joint photo, and got a seal and signature from my employer. He told me to submit the documents at the nearby ESI office. Can I go alone and submit the documents, or do I need to call my parents too? The ESI office is near Ameerpet, Hyderabad. I don't know on what day and at what time I can go there. Also, to whom do I have to submit the documents over there? Please, someone, suggest to me.

How can I get an ESI Smart Card for employees? I don't have any information about that. Could you please let me know where I can reach and how to collect ESI Smart cards? I have our employees' e-pehchan card and Medical Acceptance card.
