Ojt Program Evaluation Form - Doc Download

Hi HR Partners!
Good day to all! I am new in this site/group and I am seeking for your professional expertise to help me arrive with a training questionnaire where an on-the-job trainee (student) could evaluate the training program (company) that they had in the company.
We would like to enhance our OJT program for the students in a biomedical industry for us to be able to provide future student trainees a better OJT experience and program and also to strengthen our partnership with the academe.

Above all documents are for ILT training not for OJT. OJT sheet should consist lot more parameter & much more detailed information about Job performed
