LTI,MTI,Frequency Rate

Dear All
I wanna know about the definition of LTI,MTI and also required how calculate severity ranking and frequency rate.

Lost Time Incident means any incidents resulted in a person cannot perform his duty for one or more shift(Excluding the day of incident).
Medical Treatment Incident means any incident required medical attention for the IP by a medical practioner(Including prescribed medication) other than the onsite First Aid Treatment.
