we usually do RCA with categories of man, material, machines viz 5M's.
this may not be suitable for all industries - for your service industry, you are not dealing with any machines, materials.
you can customize in following way:
lack of standards - work instructions
lack of procedures
lack of training skills
lack of competencies
lack of specified formats / templates / documents
lack of processes - sequential operations
lack of systems - relevant IT tools / packages
resource constarints
no check points - milestones / major operation reviews etc.
