Need A Simple Procedure For Making Appraisa Companyls In My

hi, I am the only HR in my company. My boss give me a responsiblilty to do the appraisals on behalf of employees attendence,their working hours and discipline.Can anyone please suggest me,what should be the first step for this? what should be the pocedure for this??

Dear Sahay,
This is only a simple format. In this form item 6 to 14 & 16 to be filled by the person who supervises the particular employee. And items 1 to 5 and Leave/Absent to be filled by personnel/HR. If monthly Leave/Absent particular arefilled, Item 15 will automatically filled by itself.
