Thank you for posting the questionnaire. Here are my comments and suggestions.
It is futile to conduct such a specialist survey at a forum for HR personnel. I do not think you will get many responses from the sector you are trying to target. Also, do not expect people to download your questionnaire, answer, save and then reattach it here or send it to you as an email. Instead try to use a web-based service.
Now, some specific comments about the questionnaire design. Please see my today's posts which have a link to questionnaire design. Did you pilot your questionnaire before posting? Nothing more annoying than to find that one has to give personal details at the outset, when one does not know how it will benefit the respondent. hence, kindly move demographic details to the end.
Questions 11 and 16 are suitable for "Yes", "No", "NA" responses. Q19 assumes that one takes frequent leave. Q21 combines two aspects "are/could be adopted"; how will you know which one had the respondent in mind? Q22 assumes that the respondent works in a team.
