Proposal For Employee Welfare Forum - Pls Help

HI,
Our Company has decided to start a welfare forum. In which the employees can share their grievances. So i was asked to prepare a proposal for the same Any one having previous experience regarding this, pls forward me the details orelse give me some inputs so that we can modify.

initially you would be getting superficial/ cosmetic feedback, handle it for the first few months. everybody would be gauzing the seriousness of mgmt over this issue.
remember one thing:
a) acknowledge each and every suggestion that you receive
b) Think of ways to say YES (normally mgmt wants to maintain status quo and procrastinates over trivial matters eg plumbing/ electrical installatons etc).
c) if possible initaite an award for good performance amongst variousdomains, the award can be given quarterly, one for each process/ work area.
