Ways To Improve Adminstration Department- Questionaire.

Hi everybody,
I am looking forward to create a strategy for further/better improvement in administration department. My organisation offers information, support and referral to other agencies to parents who are having relationship difficulties. Manual Filing system is very important in such organisations.
I need in help in creating Questionaire - Such as Quality measures:
In what way can admin dept be improved?
How to measure adminsitrators performance- what sort of questions must be included in the performance evaluation.
Please assist me in developing a general questionarie what points should be focued?
