It starts with goal setting - individual goals that help attain Department goals, which are part of the overall organizational goals embodied in the Mission Statement (the organizational roadmap -
"We are [or want to be] the premier advertising agency in the [Region/State/Country/World] by meeting or exceeding the expectations of our clients, developing timely, quality, and high impact presentations, while providing open communications, fair and responsible treatment of all employees in a safe, healthful and enjoyable working environment.")
Since you have a project team, each on should be assessed on their particular contribution from project leader copywriter, to graphic designer, to the rest of the team the focus should be on responsibilities and results with a team evaluation (in writing of all other members) as to how the goals were/ were not accomplished (individual responses), the reasons and corrective action deemed necessary.
